Performing a Root Cause Analysis (RCA) is both a science and an art.

  1. Classify and localize the problem,
  2. determine how often the problem is detected,
  3. observe the problem under initial conditions,
  4. determine under which conditions the problem occurs,
  5. change factors around the problem area,
  6. turn the problem off and on, the corrective action should not cause other problems,
  7. test the proposed corrective action,
  8. record lessons learned and
  9. implement solution elsewhere, corrective actions, conclusion applicable across industries.

ISO 9001 in developing economies
